To give the best possible care to our patients, we reserved time especially for you. We appreciate your promptness and your consideration in not changing your scheduled time. However, if you need to change your appointment, a 24-notice is expected, or a $30 charge may apply.
A deposit fee of $100 is required in order to reserve the appointment for a sedation visit. This fee will be reimbursed when the appointment has been completed. The deposit fee will not be reimbursed if the appointment is broken or not canceled at least 2 business days prior to the appointment.

We will accept other insurances, and as a courtesy, we will submit claims to your insurance. You are, however, responsible for any fee that is not reimbursed by your insurance company. All remaining fees are expected at the time the service is provided.
Terms of Payment
All payments are due at the time of service, unless previous arrangements have been made. We accept cash, Visa, and MasterCard.
